APPLICATION FOR SKATING COURSE 2024/2025

I am registering the person listed below for a skating course:

Figure Skating Skills:

Beginner(I can't skate yet, or I can do basic moves like “sausages” – forward pushes, basic glides, simple steps)

Intermediate(I can do crossovers, three-turns, Mohawk steps, single Salchow, and upright spin)

Advanced(I can connect skating elements, perform changes of direction and edge, single jumps, and various types of spins)

Course Terms and Conditions

The courses are organized by the club Akademie Tomáše Vernera z.s..All lessons take place at the Icerink Ice Stadium, Přetlucká Street, Prague 10 – Strašnice, and are led by qualified coaches and their assistants.

The course price list includes 30 lessons held from September 2025 to April 2026.

The course fee covers 30 lessons.The course must be paid before the first lesson to the account number 620 620 6220 / 5500, using the variable symbol: 20252026.

The total amount, which is due by the end of September 2025 at the latest, can be split into two payments, each covering 15 lessons. The second payment must be made before the 16th lesson.

The price does not include accident insurance or any other type of insurance. As organizers, we are not responsible for the health condition of the skater.

Skaters are not allowed to enter the ice if the ice resurfacing machine is operating or if a coach is not present.Missed lessons are not refunded or replaced, except in cases of absence due to health reasons exceeding 3 lessons, and only upon presentation of a doctor’s note. Other reasons for absence will not be considered.

Off-ice training: 4,500 CZK

Solo Ice Dance: 13,500 CZK

2 × Adult course: 22,500 CZK

2 × Off-ice training: 8,500 CZK

Adult + Solo Ice Dance: 24,000 CZK

2 × Adult course + Solo Ice Dance: 35,500 CZK

2 × Child course: 17,000 CZK

Individual lessons:

ADULT: 450 CZK

SOLO ICE DANCE: 500 CZK

CHILD: 350 CZK

Individual lessons (with refund option*)

ADULT: 350 CZK

SOLO ICE DANCE: 400 CZK

CHILD: 250 CZK

For the duration of the course, I also agree to sending information about the following courses to the address I have provided. I agree to the taking of photographs and video recordings, to the processing of personal data to the extent of: the child's appearance (photograph, video recording), name, surname within the meaning of Act No. 101/2000 Coll., on the Protection of Personal Data, as amended, as well as Regulation (EU) 2016/679 of the European Parliament and of the Council, General Data Protection Regulation, effective from 25.5.2018, for the purposes of recording the course of the event and for the purposes of promoting the administrator. I further consent to the publication of these records on the administrator's website, the administrator's profiles on social networks, and promotional materials.
